Regular S/N Voucher Application
Instructions
The applications will need to be emailed to [email protected]
These three items MUST be included in the reply email to receive a voucher:
- Fully filled-out voucher application
- Photocopy of your ID (and proof of residence, if needed)
- Photocopy of your method of qualifying
Directions:
- Fill out the attached voucher and save it. Reply to this email and attach the filled-out application.
- Make a photocopy of your photo ID and attach it to the reply email. (Must be readable)
If your address is not correct on your ID, also send a readable photocopy of something that is addressed to you at your current address. (mail, lease agreement, etc.) - See page 2 of the application to determine how you qualify.Include a photocopy of the method you qualify with. (Lonestar, Proof of Income, top sheet of last year’s tax return, etc.)
PLEASE NOTE: All attachments need to be either JPG or PDF only.
Once the application and all other needed attachments are received, we will set up a voucher and email it back to you.
You will need to print the voucher out and take it to your Nacogdoches veterinarian in order to use it.
TNR Voucher Application

The applications will need to be emailed to [email protected]
- Fill out all applicable areas.
In the animal section, fill in as much information as you know. We mainly need to know color and breed (long hair or short hair).
It’s ok if you don’t know the sex or have a name for the animal.
You will also need to read and sign the second page. - When you return the filled-out form, also attach a readable photocopy of your photo ID.
If your address is not correct on your photo ID, please also attach a readable photocopy of something that is addressed to you at your current address. (mail, lease contract, etc.)
PLEASE NOTE: All attachments need to be either JPG or PDF only.
Once the application and all other needed attachments are received, we will set up the voucher(s) and email it to Southside Animal Clinic.
You will need to call Southside Animal Clinic (936.564.4422) and set up appointments for you to bring in your trapped feral(s).
Note: The feral cats must be brought into the vet clinic in live traps, no kennels.
